  2. Personal Data Sheet (PDS) (CS Form No. 212, Revised 2017) (original copy), which can be downloaded from: (http://www.coa.gov.ph/phocadownload/userupload/downloadable-CSC forms/PDSRev2005.pdf)
    Please follow guidelines on how to accomplish the PDS
    • Passport size ID picture
    • Government issued ID

  3. Work Experience Sheet (WES) as an attachment to the PDS (original copy)
  4. Fully accomplished affidavit on declaration of information on relatives in COA, residency status, acceptance of place of assignment and willingness to accept lower position level/salary grade (original copy) Download Here.
  5. Transcript of Records (TOR) (photocopy)
  6. Diploma (photocopy)
  7. Certificate of Training/s (photocopy) with the following conditions:

*For positions requiring relevant training
a. Must not be acquired as part of the school curriculum
b. Must be relevant to the position to be filled. A course outline may be requested by the HRMO to support the training certificate/s

  1. Photocopies of the following:

For Licensed Professionals:
(e.g. CPAs, Engineers, Architect, Teachers, etc.)
• Certificate of Registration issued by the PRC
• Certificate of board rating issued by the PRC
• Valid professional license issued by the PRC

For Lawyers:
• Certificate of membership in the Philippine BAR issued by the Supreme Court
• Certificate of BAR rating issued by the Supreme Court

  1. For CS Professional/Sub-professional:
    • Certificate of eligibility issued by the CSC

Upon Interview (After taking the COA Pre-employment Test):

  1. Original copies of the following documents for the verification of the photocopies submitted:
    • TOR
    • Diploma
  2. Decision issued by the office/court/tribunal for appointee with decided administrative/criminal case (photocopy)

 Additional Requirements for Transferees:

  1. Last approved appointment (photocopy)
  2. Performance Rating/s in the last rating period (photocopy)
  3. Clearance from the Office of the Ombudsman (photocopy)

 Additional Requirements for Reemployment:

  1. Clearance from previous employer-government agency (photocopy)
  2. Clearance from the Office of the Ombudsman (photocopy) After the Interview (before RSPB deliberation)
  3. Notarized PDS (2 original copies)
  4. Updated WES (2 original copies)
  5. Notarized affidavit (original copy)
  6. Certificate of employment (original copy)
    *For positions requiring relevant experience
  7. Certificate of Live Birth (original copy issued by the PSA or the LCR of the municipality or city where the birth was registered)
