Recruitment and Hiring Process
Interested applicants must submit to COA Regional Office No. VI, Ungka 1, Pavia, Iloilo Receiving Section the following documents (Before taking the COA Pre-employment Test):
- Application letter
- Personal Data Sheet (PDS) (CS Form No. 212, Revised 2017) (original copy), which can be downloaded from: (http://www.coa.gov.ph/phocadownload/userupload/downloadable-CSC forms/PDSRev2005.pdf)
Please follow guidelines on how to accomplish the PDS
• Passport size ID picture
• Government issued ID
• With thumb mark - Work Experience Sheet (WES) as an attachment to the PDS (original copy)
- Fully accomplished affidavit on declaration of information on relatives in COA, residency status, acceptance of place of assignment and willingness to accept lower position level/salary grade (original copy) Download Here.
- Transcript of Records (TOR) (photocopy)
- Diploma (photocopy)
- Certificate of Training/s (photocopy) with the following conditions:
*For positions requiring relevant training
a. Must not be acquired as part of the school curriculum
b. Must be relevant to the position to be filled. A course outline may be requested by the HRMO to support the training certificate/s
- Photocopies of the following:
For Licensed Professionals:
(e.g. CPAs, Engineers, Architect, Teachers, etc.)
• Certificate of Registration issued by the PRC
• Certificate of board rating issued by the PRC
• Valid professional license issued by the PRC
For Lawyers:
• Certificate of membership in the Philippine BAR issued by the Supreme Court
• Certificate of BAR rating issued by the Supreme Court
- For CS Professional/Sub-professional:
• Certificate of eligibility issued by the CSC
Upon Interview (After taking the COA Pre-employment Test):
- Original copies of the following documents for the verification of the photocopies submitted:
• TOR
• Diploma - Decision issued by the office/court/tribunal for appointee with decided administrative/criminal case (photocopy)
Additional Requirements for Transferees:
- Last approved appointment (photocopy)
- Performance Rating/s in the last rating period (photocopy)
- Clearance from the Office of the Ombudsman (photocopy)
Additional Requirements for Reemployment:
- Clearance from previous employer-government agency (photocopy)
- Clearance from the Office of the Ombudsman (photocopy) After the Interview (before RSPB deliberation)
- Notarized PDS (2 original copies)
- Updated WES (2 original copies)
- Notarized affidavit (original copy)
- Certificate of employment (original copy)
*For positions requiring relevant experience - Certificate of Live Birth (original copy issued by the PSA or the LCR of the municipality or city where the birth was registered)
- Marriage Contract/Certificate – for married applicants(original copy issued by the PSA or the LCR of the municipality or city where the marriage was registered)
- Decision issued by the office/court/tribunal for appointee with decided administrative/criminal case (certified true copy)
- Original authenticated copies of the following:
For Licensed Professionals: (e.g. CPAs, Engineers, Architect, Teachers, etc.)

- Medical Certificate (CS Form No. 211, Revised 2018) (original copy)

• Blood Test
• Urinalysis
• Drug Test
• Chest x-ray - Valid NBI Clearance (original copy)
- Original Result of Neuro-Psychiatric Examination Upon Assumption to Office
- Certification of Assumption to Duty (CS Form No. 4, Revised 2018) (2 original copies)
- Oath of Office (CS Form No. 32, Revised 2018) (2 original copies)
- Sworn Statement of Assets, Liabilities and Net Worth (2 original copies)
- Position Description Form (DBM CSC Form No. 1, Revised 2017) (2 original copies)
- Integrity Pledge (original copy)
Additional Requirements for Transferees:
- Clearance from previous employer (original copy)
- Clearance from the Office of the Ombudsman (original copy)
- Authority to transfer from previous employer (original copy)
- Certification of last salary received (original copy)
- Certification of leave (original copy of the form provided by the COA HRMO) for transferees from other government agencies.
